• 2012-2013 Membership Application - have you joined BPES PTA yet? If not, here's the form!
• 2012-2013 Volunteer Application - if you plan on helping in your child's classroom in ANY WAY (including field trips, room parties, special grade level activities, etc), then this form must be completed and turned in.
• Request for Payment (Disbursement Request)
