Useful Forms

Here's some useful forms that you might need!

• 2010-2011 Membership Application - please show your support by joining BPES PTA. The number one reason to join the PTA is to benefit your child. By doing so, you also help you child's school.
• 2010-2011 Volunteer Application - if you plan on helping in your child's classroom in ANY WAY (including field trips, room parties, special grade level activities, etc), then this form must be completed and turned in.
• 2009-2010 Spirit Wear Order Form - every Friday at BPES is SPIRIT DAY! Show your school spirit and wear BPES t-shirts, sweatshirts, backpacks.... anything with BPES on it! Tie-dyed t-shirts and solid drawstring backpacks are for sale from the BPES PTA. $12 per shirt, $5 per backpack.
